Hyde Park Pediatrics is proud of our designation as a Patient-Centered Medical Home through the National Committee for Quality Assurance. This program ensures that we put your child and your family at the center of all we do! We work as a team to ensure that we help your family meet your goals for your child’s health, that we are in close communication with the specialists and emergency department providers that your child sees, that we meet your needs for scheduling appointments and providing after-hours advice and care, and that we are setting goals for quality improvement on patient care measures. We routinely seek your feedback and make changes based on your ideas. Lastly, we provide care management for patients with certain medical and behavioral conditions.

For our patients with more complex health needs we have a program called PACC (Pediatric Alliance for Coordinated Care). This program was initiated by Boston Children's Hospital to assist pediatric practices in caring for the most complex patients in their own community.  The PACC Program, run by Debra Simons, one of our nurse practitioners, is for select families who have children with complex medical needs.

The program enables us to provide extra resources and oversight to help with management of complex medical issues, specialist appointments, durable medical equipment, letters of medical necessity, medication review, and any other support that is needed. Helping families with these concerns and crystallizing each medical issue enables better and more comprehensive care of these patients which hopefully leads to healthier outcomes and decreased family stress.
