Before your visit
We look forward to your visit at our office!
There are forms that you can download and fill out prior to coming to the office. Filling these forms out at home gives you the chance to think about questions prior to your visit and help us serve your needs. (Please fill out a new Demographic Sheet whenever you change address or have a change in your household.)
Some general tips to help you get the most out of your child’s encounter in our office:
- Write down your questions prior to your visit.
- It is also necessary for you to be on time for your visit, this helps us stay on time. If you are running late, please call our office so we may reschedule your visit if need be. If you are more than 15 minutes late for your appointment, we will reschedule your child to our next available appointment.
