For new patients transferring into the practice, please forward a complete copy of your child’s medical record prior to your first appointment. You may download a Records Release Form. You can then make an appointment with one of our five pediatricians. If your insurance plan requires you to chose a primary care provider, please contact your insurance company and select one of our physicians as your child’s primary care provider.
Please bring your insurance cards, drivers license and co-pay with you for all of your children’s visits. Please download our new patient registration forms (below), complete them prior to your visit and give them to the receptionist when you check in.
Please plan to be on time for your appointment. If you are 15 minutes late, we will have to reschedule your appointment. Co-payments are due at time of visit. For your convenience we offer payment by cash, check, MasterCard, Visa and Discover card. If you do not have your co-payment at time of visit, you may be asked to reschedule your appointment.
