Medical Home
In 2010, we adopted the American Academy of Pediatrics’ Medical Home model of care. Our Medical Home Care Coordinator (MHCC) leads our efforts to continually improve our patients' experience and the quality of care we deliver. Our MHCC will help with follow-up for patients with complex medical, behavioral and socio-economic needs. You may receive phone calls and/or letters through MyChart from our MHCC, George MacDonald, RN.
If your child has special health care needs, you may contact our MHCC at 617-277-2541 for assistance in coordinating your child's care. Among other tasks, she can help you schedule visits with specialists and find community-based organizations or resources that will be beneficial to you or your child.
