Please be sure to bring your child’s insurance card to each visit. You are responsible for providing us with your child’s current insurance information so that we can accurately bill each office visit.
We are contracted with most of the major health insurance carriers, but it is your responsibility to contact your insurer to make sure we are listed on your plan. Please feel free to contact our office with any questions regarding insurance or payment plan options.
Copayments: Please be prepared to pay your copayment at the time of your visit. We accept cash, checks and credit card payments.
