We welcome you to our practice! We would like to thank you for choosing us and we look forward to being a part of your child’s life!
Below are a few items that may be helpful in preparation for your first visit with us. Again, call us if you have any questions or concerns. We look forward to meeting you and your child!
Expecting a new baby?
Congratulations! Having a baby is one of the most exciting times in your life. We know that choosing the pediatrician who can best care for your child is an important decision. If you would like to meet one of our doctors prior to the delivery of your baby, we offer complimentary Pre-Natal visits. This allows the expectant parents to meet the pediatrician and learn about the practice. If you would like to meet one of our doctors prior to the delivery of your baby, please call our office at (508) 999-2981 and indicate that you would like to schedule a Pre-Natal visit.
If your child is born at St. Luke’s Hospital, your baby will be cared for by one of our pediatricians. At the time of discharge, the doctor will let you know when your baby should be seen in our office for follow-up. Whether or not your child is born at St. Luke’s or another hospital, please call our office at discharge at (508)999-2981 to arrange their first appointment.
Please remember to notify your insurance company following the birth of your child so that the new baby will be added to your insurance coverage. Contact our Billing Department at (508)910-3374 as soon as you receive your child’s policy information so that we may send out any pending insurance claims in a timely matter.
Note regarding MassHealth coverage: Our practice only accepts the WellSense Boston Children's ACO plan through MassHealth. This plan is only offered to children. If Medicaid coverage is present the child’s parent/guardian must contact MassHealth and select the WellSense Boston Children's ACO plan in order to receive covered medical services from our practice.
When you arrive at our office
Our office consists of two levels, upper and lower. A directory of providers is located in our lobby. Please sign-in with the provider’s secretary upon arrival. You may need to complete some paperwork such as a Patient Registration Form, HIPAA Privacy Practices and a patient and/or family history. Please bring your child’s valid insurance card at each and every visit.
New to our practice?
We ask that you please arrange to have your child’s medical records including immunizations from their previous physician transferred to our practice prior to their first Well Child Visit. We have medical release forms available in our office. Also, please make sure that you contact your insurance company to change your child’s PCP before coming in for your visit.
When you are checking out
Please stop to check out with one of our secretaries before leaving. The provider’s secretary will arrange any diagnostic tests, referrals or follow-up appointments. Some insurance plans require a referral if your child needs to see a specialist. Referral requests for additional visits should be made at least 48 hours prior to the appointment with the specialist.
